History Questionnaire – For new patients, we require a completed History Form prior to booking an appointment. Our team of Client Service Counselors can walk you through the steps for booking a new appointment, and they will provide a link to complete the Form online. If you would like to familiarize yourself with the types of questions asked, a sample form is included here for reference. (Note: this is a sample form only, and is not intended for client use)
Vet-to-Vet Phone Consultations – These consults are conducted between one of the Animal Behavior Clinic’s veterinarians and an external veterinarian via phone to discuss possible treatment and/or medication strategies for their cases. Availability for v2v consults is typically in the same week as requested, making this an ideal option for urgent cases. Cost is charged directly to the veterinary clinic.
